Carson Tahoe Center for Wound Healing Launches Nevada’s 1st ‘Integrated Wound Care Community’ Program at Ormsby Post Acute Rehab and Carson Nursing and Rehabilitation Center
The Carson Tahoe Center for Wound Healing has launched an innovative Integrated Wound Care Community (IWCC) program at Ormsby Skilled Nursing Facility and Carson Nursing and Rehabilitation Center. Designed to ensure seamless continuity of care, reduce the likelihood of hospital readmissions, and decrease length of stay in the acute care setting, this pilot program allows care providers to come directly to the patient. Carson Tahoe is the first provider in Nevada, and one of only eight Centers in the country, to participate in this leading-edge program. Dr. David Johnson, Medical Director of the Carson Tahoe Center for Wound Healing in Carson City, is overseeing the program.
Through the IWCC initiative, patients are immediately given an individualized wound healing plan of care, beginning in whatever setting they use to access the healthcare system; this personalized plan follows them throughout the course of their care, whether they are in an acute, post acute or outpatient facility. Dr. David Hoskins, Medical Director of the Carson Tahoe Center for Wound Healing, Reno, along with Rachael Wilson, PAC and Kelly Nalder, APRN regularly visit these patients in various care settings, and provide treatment for them as part of the program.
“The IWCC program allows us to treat patients across the full continuum of care,” said Dr. Hoskins. “By taking this proactive, patient-centered approach, one that includes visiting the patient in their specific care setting, we can help prevent hospital readmissions, reduce length of stay, and support the best possible outcomes; we are essentially bringing personalized care directly to patients, reducing the need for them to access the hospital. We’re also pleased to work as a partner with the Ormsby Post Acute Rehab and Carson Nursing and Rehabilitation Center teams to help with staff support and development needs.”
Prior to becoming the Medical Director of the Carson Tahoe Center for Wound Healing, Reno in 2015, Dr. Hoskins served as a primary care physician in our community since 1983. For many years, he was the only physician who visited and treated nursing home patients in the Douglas County and Carson City area. Dr. Hoskins is passionate about helping chronic wound patients heal, allowing them to enjoy the best possible quality of life.
